Eligibility for medical plan offerings at IU Health will be determined by the county in which you reside. Employees who reside in any of the counties* well represented by the expanded network of IU Health are eligible for Quality Plan I – Select Plan, Quality Plan II (where care may be obtained from a provider or facility in a broad national network) or Quality Plan I – High Deductible Health Plan (HDHP) coupled with a Health Savings Account (HSA). Employees who are residents of other states or who reside in Indiana counties not listed below are only eligible to enroll in Quality Plan – Out of Area (plan design and premium match Quality Plan I – Select and the broad national network of Quality Plan II) or Quality Plan – HDHP – Out of Area (plan design and premium match Quality Plan I – HDHP and the broad national network of Quality Plan II).
* Covered Indiana counties: Benton, Blackford, Boone, Brown, Carroll, Clinton, Delaware, Hamilton, Hancock, Hendricks, Howard, Johnson, Lawrence, Marion, Martin, Monroe, Morgan, Owen, Tippecanoe, Tipton, Vermillion, Vigo and White.
