Diabetes is the top disease that leads to chronic kidney disease and renal failure. Complications from diabetes can be serious – and costly. HealthSmart’s Diabetes Education Course tackles the rising healthcare costs associated with diabetes through intensive and interactive education.

This proprietary course is specifically tailored for members with type 1 and type 2 diabetes using the “teach-back” methodology. Weekly assignments are given so instructors can ensure and confirm that the concepts are received and understood. Classes are available weekly in the evenings, and can be accessed from any location in a webinar setting.

  • Engages and educates newly diagnosed members which mitigates costs due to complications.
  • Members receive clinical assessments from a registered nurse, a consultation with a registered dietitian and an activity assessment from a certified wellness coach
  • Presentations by a board-certified endocrinologist, internist, and registered pharmacist
